RESOURCE NAVIGATION TO BUILD A NETWORK OF SUPPORT AROUND FAMILIES

The Patient Advocacy Program of the Olivia Caldwell Foundation provides resource navigation to Wyoming families when they have a child or children who have been diagnosed with a chronic or complex medical illness. Our goal is to support the child (and their entire family) by building a network of support with our resource partners and providers across the state to meet as many of the family's needs as possible.
Our Patient Advocate simplifies the referral process to other nonprofit agencies, state agencies, welfare services, home health care agencies, counseling services, student services, and nearly anything else. We do the legwork, knowing which services will be the best fit based on your family's specific needs so you don't have to do the research when your whole life is falling apart! Our Patient Advocacy Program for resource navigation is available at NO COST to families statewide regardless of income level or type of diagnosis. |
